Acknowledgment of Risk
I understand that participation in dance classes, summer camps, intensives, workshops, and related activities at Archer Classical Training involves physical activity and inherent risks, including but not limited to injury, muscle strain, falls, or other physical harm.
I acknowledge that these risks may arise from participation in ballet technique, conditioning, stretching, choreography, and other training activities.
Assumption of Responsibility
I voluntarily choose to participate (or allow my child to participate) in programs offered by Archer Classical Training and assume full responsibility for any risks, injuries, or damages that may occur as a result of participation.
Release of Liability
I, on behalf of myself and/or my child, hereby release and hold harmless:
Archer Classical Training, ArcherCo LLC, its owners, instructors, staff, and the facility in which classes are held
from any and all liability, claims, demands, or causes of action arising out of or related to participation in any classes, camps, intensives, or workshops.
Medical Consent
In the event of an emergency, I authorize Archer Classical Training to obtain medical treatment for myself/my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.
Media Release (Optional but recommended)
I give permission for Archer Classical Training to photograph or video my child for promotional and educational purposes.
Health & Wellness
I confirm that I/my child is physically able to participate in dance activities and has no medical conditions that would prevent safe participation. I agree to inform staff of any relevant injuries, conditions, or limitations.
Program Acknowledgment
This waiver applies to all Archer Classical Training programs, including but not limited to:
- Summer Camps
- Summer Intensives
- Workshops
- Private Lessons
Agreement
I have read and fully understand this waiver and agree to its terms voluntarily.
